Welcome to the new school year!!

School Picture Day is TODAY!!

Important information from our Health Office
Any medications provided to the health office for your child, may remain in the health office for next year unless expired.  If your son/daughter will need medication in the coming school year, a new physician’s order will be required.  (Please note that it will not be necessary to provide the school nurse with Tylenol or Advil.)

** Please be certain to provide updated orders for Inhalers and Epi-Pens and please check expiration dates for medications provided.

Information for Students ENTERING grades 6, 7, and 8:

Attention 6th Grade Parents-Important Immunization Requirements:

All 6th grade students must have documentation of two varicella (chicken pox) vaccines.  Students entering 6th grade must also receive the immunization containing tetanus toxoids, diphtheria, and acellular pertussis (Tdap).  Please provide Mrs. Costakis, school nurse, with a physician's record of this immunization by Friday, September 10th.  Also, please contact Mrs. Costakis at 871-5500, Ext. 5536 if you would like to discuss any medical issues of concern.

 Attention 7th Grade Parents- Physical Requirement: 

NY State requires all 7th graders to have a physical. The physical must be provided to the school nurse within 30 days of the start of the school year, or completed within 12 months of the start of the school year to be accepted. There is a NEW NYS HEALTH EXAMINATION FORM REQUIREMENT. This is now the only form that schools may accept, so please be sure to ask your provider to use it.   

The form will sent to you in July and is available on the school nurse’s web site as well as in most physician’s offices.

Please return completed physical forms to the school nurse.

Attention 7th and 8th grade Parents-Important NEW Immunization Requirement:  Students entering grade 7 and 8 are required to show proof of Meningococcal vaccine. Please return immunization forms to the school nurse by Friday, September 10th.  If you have concerns or questions, please contact Mrs. Costakis, RN at 845-871-5500 x 5536.
